Basic Exam Application Form

 

Class will meet on a date to be determined in early January 2010—students should have assignments prior to class.

 

 

Background Check information will be provided to students that did not attend registration session.

 

HOBETs must be completed ASAP!  Reading 42, composite 42 minimum

 

-or-

 

COMPASS/ASSET, (COMPASS Reading 40/Pre-Algebra 44, Writing sample 30) or (ASSET Reading 40, Numerical 40, Elementary Algebra 36, Writing sample 30)  *3 year limit on assessment tests.

 

We will need for you to bring for your  file:

· Copy of your current state EMT-B or EMT-I card

· Copy of your current CPR (HCP) card

· Proof of A&P completion

· HOBET if taken at another location

 

We will provide:

· Health form prior to starting clinicals (completed in prior 6 months).

· Confidentiality statement

· HIPPA agreement

· ADA agreement
