Basic Exam Application Form

 

Registration and orientation will be held on December 12, 2011. Students that have been sponsored by services or made payment arrangements will be given initial books. A short session on drug calculation will be held and a homework assignment given for completion prior to January 11.

 

Class will begin January 9, 2012—students should have assignments prior to class.

 

Background Check information will be provided to students that did not attend registration session.

 

 

We will need for you to bring for your  file:

· Copy of your current state EMT-B or EMT-I card

· Copy of your current CPR (HCP) card

· Proof of A&P completion

 

 

We will provide:

· Health form prior to starting clinicals (completed in prior 6 months).

· Confidentiality statement

· HIPPA agreement

· ADA agreement
